Nasal Hump Correction

woman after nasal hump correction procedure

INTRODUCTION

Dorsal hump excision can be accomplished through a closed or open rhinoplasty technique. The surgical approach chosen depends on the etiology of the nasal hump and other rhinoplasty manoeuvres incorporated into the procedure.

The procedure can be performed under general or local anaesthesia. In either case, local anaesthesia is injected via an intercartilaginous approach to infiltrate the lateral nasal walls. If septal and tip work are also to be performed, the septum is infiltrated, and a small amount of anaesthetic is placed between the domes and subcutaneously in the columella. In delivery or open rhinoplasty approaches, injections are made in the marginal incisions. Injections are not made in the nasal dorsum to prevent any distortion. Generally, less than 8 mL of local anaesthetic is required.

As the centrepiece of our face, the nose plays a vital role in our appearance, and it heavily influences the character of the entire face. Herewith, the nose bridge (nasal dorsum) is the largest and most prominent component of the nose and connects the tip of the nose to our face.

However, instead of a straight slope from the bridge of the nose, some individuals may have a dorsal hump that forms an enlarged bump due to genetics or an injury. A hump on the top of the nasal bridge is typically bony. A hump lower down consists primarily of cartilage.

What is a Nasal Dorsal Hump?

Nasal dorsal hump primarily refers to cartilage and bone irregularities along the nose bridge that causes the nose to have an enlarged bump. The nasal dorsal hump is a common irregularity that, in most cases, occurs naturally. In general, it is not dangerous or associated with any health issues. However, most people with a dorsal hump wish to correct it because the enlarged hump can be aesthetically unpleasing, particularly when viewed from the side.

Shens Clinic offers several options for dorsal hump correction. Dr Shens has extensive experience in cosmetic procedures, making him able to address this deformity both surgically and non-surgically. Here you will find some additional information about the dorsal humps and how they can be corrected.

What causes a nasal dorsal hump?

The nasal dorsal hump is most often caused by:

  • Genetics. The irregular bridge is often a genetic trait. If your parents have humps, it's likely you will have one too.
  • Trauma. A broken or crooked nose could result in a deformity and the appearance of a dorsal hump.

Among the rarer reasons for developing a bump on the bridge of the nose are:

  • Severe nasal infection. It is possible to develop a dorsal bump as a result of severe bacterial or fungal infection. However, such cases are quite rare.
  • Bad allergies or nasal polyps. Such medical conditions may sometimes cause cartilage and bone irregularities in the nose.

A dorsal hump is usually just a normal anatomic variation, a natural shape that somebody has inherited. The bump may appear in the nostrils, nasal septum or in the nasal cavity. In any case, such a deformity is not seen as attractive by many people. It often looks like a broken nose.

What are the options for removing the dorsal hump?

The nasal dorsum can be easily corrected through some cosmetic procedures. Shens Clinic offers surgical removal of the dorsal hump, either through open rhinoplasty or closed rhinoplasty, as well as non-surgical correction through dermal fillers.

Open rhinoplasty

open rhinoplasty

Open rhinoplasty is the traditional nose surgery method used by most plastic surgeons. The procedure is done through an external incision at the base of the nose (columella). By making such an incision, the plastic surgeon is able to open the soft tissue envelope and gain better visualisation of the nasal framework. The upper lateral cartilages are separated from the spectrum and corrected in order to give the nasal bridge a straighter line.

While this surgical technique places a small scar across the columella, the incision often heals well and is not noticeable at conversational distance.

Closed rhinoplasty

closed rhinoplasty

Closed rhinoplasty, also so-called scarless rhinoplasty, is a nose surgery where the plastic surgeon makes all of the cuts inside to elevate the skin from the nasal framework. Thus, changing the nose shape. This procedure is primarily developed to fix broken noses and minor reshaping, where more work on the nose tip need not be carried out.

One of the biggest advantages of closed rhinoplasty is that it doesn't leave visible scars since there are no external incisions involved. More people choose this approach over the open method when it comes to nose reshaping.

Non-surgical rhinoplasty, also known as liquid rhinoplasty, is a cosmetic procedure that uses dermal filler to correct the nasal hump. This method involves injecting a substance into different nasal regions to camouflage the hump.

One of the most commonly used materials is Hyaluronic acid, which is a natural substance. However, liquid rhinoplasty does not change the structure of the nose but makes it visually look more contoured. In addition, the nose filler injection is only a temporary solution. If you want to maintain your results, you will need to repeat the procedure after one or two years.

Open vs closed rhinoplasty: Which is better?

Many patients who have a nasal hump are interested in a nose job. Hence, one of the most common questions is what kind of plastic surgery they should get - open or closed rhinoplasty. Typically, a closed rhinoplasty approach is preferred by patients because there are no visible scars compared to an open rhinoplasty procedure.

What are the other differences between the two types of rhinoplasty, and what are the advantages of each? Let's find out.

Open rhinoplasty procedure

  • A small incision is made on the external surface of the nose
  • The nasal tip cartilages are easier to visualise
  • The plastic surgeon has a direct view of the inner structures

Advantages of open rhinoplasty

  • This surgical procedure allows more precise intraoperative diagnosis
  • There is less distortion of the nasal framework from intraoperative retraction
  • It is ideal for treating complex nasal deformities
  • Ideal for the second or third revision through which extensive scar tissue and cartilage grafting techniques are necessary

Disadvantages of open rhinoplasty

  • There may be visible scarring after the procedure
  • Recovery time tends to be longer with this procedure

Closed rhinoplasty procedure

  • It is performed through internal incisions
  • The bone and cartilages are reached through nasal passages

Advantages of closed rhinoplasty

  • There are no scars on the external part of the nose
  • The operative time is usually shorter
  • The recovery is easier and faster

Disadvantages of closed rhinoplasty

  • The nasal framework cannot be seen very well
  • The technique is suitable only for minor nose reshaping

What type of cosmetic surgery is best for you will depend on many factors, such as the degree of nasal irregularity, the type of nose shape you desire, the rest of your facial features, and so on.

What is the recovery from rhinoplasty?

The recovery time for scarless rhinoplasty is one to two weeks. Most patients can return to normal activities after this time, including work, social activities, and public appearances.

A small amount of swelling and bruising may occur in the nasal region in the first two to three days after the cosmetic surgery. Also, you may experience mild discomfort in the area of the incisions, but it will quickly disappear.

The recovery from open rhinoplasty can take anywhere from two to six weeks. You are more likely to experience swelling and bruising in your nose area, which can last longer than a month. There may also be some swelling around your eyes.

After nose plastic surgery, you will need to wear a nasal splint for the first week to ensure your nose heals properly. Your doctor may also prescribe you some oral and topical medications. You will also receive special instructions on how to properly take care of yourself so that your recovery goes smoothly.

What are the side effects of rhinoplasty?

Nose surgery is a relatively safe procedure that rarely causes serious complications. Medical complications are quite uncommon. Nevertheless, it is imperative that you are aware of all possible risks before having this plastic surgery.

Most patients experience just mild to moderate swelling, bruising and discomfort following nasal hump surgery. A few other side effects may also occur, including:

  • Bleeding
  • Infection
  • Pain
  • Scarring
  • Breathing issues
  • skin sensation changes
  • Damaging of the nasal septum
  • Poor healing of the incisions
  • Unsatisfactory outcomes and the need for revision rhinoplasty cartilage grafts.

It is critical that you follow the surgeon's pre-op and post-op instructions precisely so that you reduce the risk of experiencing adverse side effects.
