Saline breast implants are filled with a sterile saltwater solution, which is safe for the body in the event of an implant leak. They are also less expensive as compared to silicone implants. Breast augmentation is typically performed as an outpatient procedure, meaning you can go home the same day. Here’s what to expect on the day of surgery:
You will have scheduled follow-up appointments to monitor your recovery. These check-ins allow the surgeon to assess the healing process and ensure that the implants or fat grafts are settling as expected.
The breast implants can be placed either above or below the chest muscles (pectoralis major muscles). The breast implants can be inserted through an incision from either the underarm (axillary), around the nipples (peri-areolar), or under the crease of the breast (inframammary). Patients can also opt for laser resurfacing to reduce/ lighten the scar. The plastic surgeon would advise the most suitable approach based on the existing breast shape of the patient.
Three types of incisions that can be used for breast implant surgery:
The periareolar incision is made around the areola, the darker skin surrounding the nipple. This method is favoured by many because the scar can blend into the natural transition between the areola and the breast skin, making it less noticeable. It provides better access for the surgeon, allowing precise placement of the implant. This technique is often used when a breast lift is performed in conjunction with augmentation. However, it might affect breast feeding and has a slightly higher risk of affecting nipple sensation.
The transaxillary incision is made in the natural crease of the armpit. A key advantage of this approach is that there are no scars on the breast itself; scars are hidden within the armpit. This technique involves the use of specialised instruments and a camera to assist in placing the implant correctly. It can be used for placing either saline or silicone implants, and is particularly popular among patients who want to avoid scars on their breasts. However, repeat surgeries may not always be able to use the same incision site.
The inframammary incision is made in the crease under the breast, known as the inframammary fold. This is the most common technique used for breast augmentation as it provides the surgeon with direct access to the desired area under the breast, allowing precise placement of the implant. The scar, typically about an inch long, is well-hidden under the fold. This method is versatile and can be used for all types of implants. It is especially beneficial for larger silicone implants and generally has minimal impact on breastfeeding. The under-pectoral muscle incision (along the fold of the breast) is one of the most preferred methods, as it allows a short incision point and hides the scar well.
After the breast augmentation procedure, patients will experience soreness and swelling in the chest area. These symptoms will subside a few weeks after the surgery and the overall recovery period will take roughly 1-2 months. During the healing process, patients may be advised to wear compression wear or a sports bra for additional support for the breast implants. Patients with non-strenuous job activities should be able to return to work within a week. Capsular contracture is one of the most common risks associated with breast augmentation. It occurs when scar tissue forms around the breast implants, leading to tightness, discomfort, and a distorted appearance. It can be uncomfortable and may require additional surgery to correct.
Infection is a potential risk following any surgical procedure, including breast augmentation. Although rare, infections can occur after breast augmentation surgery and may require antibiotic treatment or, in severe cases, implant removal. Patients should follow essential measures, such as maintaining proper hygiene, avoiding exposure to potential sources of infection, and following your surgeon's guidelines for post-operative wound care. By taking these precautions, you can significantly reduce the risk of infection and ensure a smooth recovery.
While implant rupture is relatively rare, it is important to understand the causes, signs, and potential risks associated with this complication. Breast implants can rupture or leak over time. If a saline implant leaks, the saline is harmlessly absorbed by the body. However, if a silicone gel implant ruptures, the silicone gel may remain within the breast or migrate to other areas, potentially causing pain, swelling, or changes in breast shape. Additional surgery may be required to remove or replace the implant.
Temporary or permanent changes in nipple or breast sensation are possible after breast augmentation. This can include increased or decreased sensitivity or numbness in the nipples or breasts.
